Assisted Living-Live In, Direct Care Professional

Title:                                                                                                               Assisted Living-Live In, Direct Care Professional

Hours:                                                                                                Shift starts Sunday 8:30am – Wednesday 8:30pm

Reports to:     AL Program Manager/ Director of Program Services

 

The Residential Service Provider reports to the Program Manager/Program Director. This person provides direct supervision, training and care for individuals served by CHAI programs in the Assisted Living Program.

 

The Residential Service Provider’s job performance will be rated on the following outcomes and performance measures:

 

Performance Measures as they relate to the Individual’s needs are:

  1. Participates in Person Centered Planning for residents and actively participates in the planning process on behalf of the resident.
  2. Conducts the resulting training and services and documents that training accurately. Reports concerns regarding training and services to the appropriate individual.
  3. Assists with transportation or training to obtain transportation.
  4. Communicates with families in a polite and respectful manner. Communicates concerns to the necessary individual in a timely manner.  Documents necessary communication in a proper manner.
  5. Follows on-call procedure to insure timely communication of emergency needs.
  6. Participates in necessary staff / house meetings and keeps apprised of resident updated information and needs.
  7. Participates in the development of daily routines with resident input and communicates changes to schedule as necessary to assure continuity.
  8. Assures residents’ privacy.
  9. Understands and respects resident’s privacy with respect to personal information, records, and confidences.  Understands where and with whom to share that information.
  10. Understands and assures that equipment and household is maintained in good repair and the process for reporting repairs and replacement of household items.
  11. Assures that all residents have full opportunity to participate in the life of their community and culture.
  12. Enables the residents to demonstrate the right to competence in their environments by being representative of a norm. That they have enough money, are on time, are dressed properly, etc.
  13. Assures that people are respected as adults and that they are free from abuses of any kind.  Staff will follow proper procedures when any abuses are noted or suspected.
  14. Notifies Program Manager/Director of any medical need.  Acts in an emergency to call 911.

 

In addition to the above, the Residential Service Provider is expected to:

 

    1. Assist residents with financial transactions as assigned.
    2. Maintain the household petty cash and make responsible budget conscious purchases for groceries and supplies as assigned.
    3. Assist with planning special CHAI events and participate in these events when they fall within their scheduled work time.
    4. Become familiar with the required standards and expectations of the programs including but not limited to: HHSC, Life Safety, Assisted Living, or Jewish Custom as it relates to the people served.
    5. Conduct any/all safety drills as required and document the results as per standards.
    6. Demonstrate competence in driving the agency vehicle and show insurability. Provide a valid Texas Driver’s License.
    7. Must demonstrate competence in all areas of required training for position such as CPR, first aid, PMAB or comparable training, Self Administration of Medication, documentation, behavioral principles, and those required by supervisor to improve performance.
    8. Must pass a physical examination and be free of infectious and communicable disease. Must be able to bend, lift, stoop and assist all residents in their care and daily living activities as a requirement of the job.
    9. Must meet and maintain a criminal history void of offenses listed on the DADS list of non-allowable offenses, offenses that represent a pattern of negligent fiduciary responsibility, or any behavior represented on the Employee Misconduct Registry

 

Qualifications:

Employees must be eighteen and have a high school education.  Experience with people with developmental disabilities is preferred.

Employees must meet and maintain criteria (6-9) above.

 

Life Skills Trainer

Title:                     Life Skills Trainer/Job Coach

Reports to:         Wolens Program Manager

 

This position works with the Wolens Program Services team to support individuals with intellectual disabilities to achieve independent living goals that are personally rewarding and/or gain meaningful employment in the community.  This position assists these individuals to attain a higher level of independence through habilitation training in their home or in the community.

 

Qualifications:

  • Bachelors degree in human services field, three years experience serving individuals with intellectual disabilities in a teaching or training capacity. Experience in assessing individuals and preparing training goals.  Must have a working knowledge of Microsoft Office Suite. DARS certification a plus. Must successfully obtain and maintain certification in First Aid, CPR, and SAMA. Have a valid Texas Driver’s License and reliable transportation.
Part-Time Direct Care Professional

This Direct Care Position is Responsible for (but not limited to):

  •  The operation, coordination, instruction and supervision of clients/residents in residential support homes or day services
  •  Ensure the protection of the individuals’ rights
  •  Assist individual clients/residents in acquiring and maintaining the self-help, socialization, and adaptive skills necessary to reside successfully in a home or a community-based setting
  • Promote a safe and healthy living environment
  • Increase the capabilities of the individual within the policies and procedures of the organization
PRN Direct Care Professional

This Direct Care Position is Responsible for (but not limited to):

  •  The operation, coordination, instruction and supervision of clients/residents in residential support homes or day services
  •  Ensure the protection of the individuals’ rights
  •  Assist individual clients/residents in acquiring and maintaining the self-help, socialization, and adaptive skills necessary to reside successfully in a home or a community-based setting
  • Promote a safe and healthy living environment
  • Increase the capabilities of the individual within the policies and procedures of the organization
Wolens Program Manager

The Wolens Program Manager supervises the employees of the Wolens Program and any contract consulting staff for that program.  The Manager is responsible for vocational, educational, and recreational activities provided to adults with intellectual disabilities served through the Wolens Program through CHAI group homes and community supports from the HCS/TXHML Program, Dallas Metrocare and private referrals.

 

REPORTS TO:      Director of Programs and Services

 

SUPERVISES:      Life Skills Trainers, Job Coaches, and Supported Employment Specialist 

 

MINIMUM QUALIFICATIONS

Education:  Bachelor’s Degree required.

Experience:  At least two years’ experience in a human service field and one year supervisory experience. DARS Certification is preferred. Working with adults with intellectual disabilities is a plus.
