A case worker from HHSC called and asked me to send in a form H1746A. What is this and where do I find it?
H1746A is a form that allows agencies to share information with MEPD workers at HHSC regarding an individual. It serves as a cover sheet and authorization for certain types of Medicaid. If you are asked for one of these forms, please contact a Benefits Specialist at the LIDDA, your Service Coordinator or your provider right away and it will be submitted on your behalf.
