Host Home & Companion Care Services
Host Home (sometimes called Host Home/Companion Care) is a residential support option where an individual with IDD in the HCS program lives in a private, home-like setting versus a larger group-home/residential environment.
CHAI contracts with more than 20 host home families, where clients receiving HCS Medicaid-waiver services are individually cared for in their homes by a designated caregiver, usually an immediate family member. Components of the host home program include:

Personal support and daily living assistance and support

Meal preparation and planning

Medication assistance (under our RN’s delegation)

Scheduling and transportation to medical appointments

User-friendly documentation software to submit daily notes and monthly fire drills
