Nursing Program

For both group home residents and host home clients, CHAI provides a comprehensive nursing program to lighten the burden of healthcare for our direct care staff (DCS) and the residents’ families. Medical assistance and healthcare management play a monumental role in the work that CHAI does to provide exemplary care. CHAI’s nursing services include medication management, pharmacy liaison services, medical appointment management and transportation, a 24/7 nursing consultation helpline, wellness checkups, and advocacy for these individuals as medical patients.

A team of contracted nurses from Nursing Innovations oversees the medical care of each of CHAI’s group home residents and host home clients, coordinating their plans of care, monitoring health conditions and risks, and training direct care staff (DCS) to properly administer medications. CHAI residents cannot manage their healthcare needs on their own, and CHAI’s nursing services ensure those needs are met.

The nursing program also includes staffing for a full-time Healthcare Support Coordinator who oversees the logistics of appointments and transportation of CHAI’s 31 group home residents to their various medical and dental appointments; this position also ensures that prescription medications are distributed to the residents in a timely manner and their guardians are provided with an after-visit summary of the appointment.

Finally, a healthcare database called Therap allows DSPs to enter every detail of each resident’s physical, medical, and behavioral health, enabling the nursing team and CHAI supervisors to promptly adapt care or intervene when necessary. Much of this data is required by the state, but much of it goes “above and beyond” the minimum data tracking that other group homes provide, enabling CHAI to sustain its exemplary care.
